From the article

Herbert took it upon himself to move the chains by scrambling on third. He was chopped down several yards short but it looked like his elbow with the ball came down right at the first down marker. He was called over a yard short and due to Harbaugh already using (and losing) his two challenges for the game, the Chargers were unable to have it reviewed... Herbert was stripped by defensive lineman Zach Allen and the Broncos recovered at the LAC 23-yard line... Herbert took the snap, looked left, then turned to his right and fired to Marquez Valdez-Scantling who was running a comeback route neat the right sideline. Herbert did not see defensive back Brandon Jones running underneath Valdez-Scantling and the pass sailed right into Jones' arms... Herbert was sacked before getting a pass attempt off, sealing their fifth loss of the year.

Herbert's 4Q collapse: scramble challenge loss, interception, strip sack, and final-play sack sealed Chargers' loss.

Herbert's fourth-quarter collapse defined the Chargers' loss: he scrambled on third down but was ruled short of the first-down marker (with Harbaugh unable to challenge due to prior usage); threw an interception to Brandon Jones on fourth-and-six when he didn't see the underneath defender on a Valdez-Scantling comeback route; was stripped by Zach Allen on the ensuing drive; nearly fumbled again on second down before recovering his own pass; and was ultimately sacked on the final play of the game, sealing the Chargers' fifth loss. The source characterizes this as a 'horrible series of events' and a 'collapse,' with Herbert limping and taking multiple hits throughout the drive sequence.
